Fix macOS "Disk Utility" Erase Failure

The Symptom

You try to format a USB drive or partition, but it fails with a generic error or "Unable to write to the last block."

The Fix

1. The 'diskutil' Wipe

Terminal:

diskutil list

(Identify your disk, e.g., disk5)

diskutil eraseDisk APFS "Untitled" /dev/disk5

1. Change the Scheme to GUID

Ensure you have selected "GUID Partition Map" in the erase dialog. "Master Boot Record" often fails on newer macOS versions.

1. Check the Physical Lock Switch

If you are using an SD card or older USB drive, check for a physical "Read Only" lock switch on the side of the hardware.

Fix 3: The Professional Drive Repair

When the drive's partition table is corrupted beyond what Disk Utility can see.
